چکیده

Heart failure is considered to be the last stage of heart disease and a significant cause of morbidity and mortality worldwide. It is characterized by the persistent activation of the neurohormonal system, endothelial dysfunction, exercise intolerance, high mortality and a poor quality of life. Exercise training has been strongly recommended as a safe and important tool for the non-pharmacological treatment of heart failure. Exercise training improves exercise capacity, quality of life, endothelial dysfunction, skeletal muscle oxidative capacity, the cathecolamine plasma level and the autonomic and ventilatory responses. In heart failure patients, designing an appropriate aerobic exercise routine is crucial for obtaining both an increase in exercise capacity and the reasonable control of exercise-related risks. Hydrotherapy (i.e., exercise in warm water) had been considered potentially dangerous in heart failure patients due to the increased venous return caused by the hydrostatic pressure. However, it is now known that cardiac function actually improves during water immersion due to the increase in early diastolic filling and decrease in heart rate, resulting in improvements in stroke volume and ejection fraction. Studies with sauna therapy (i.e., warming) have demonstrated important improvements in neurohormonal attenuation and exercise status in heart failure patients. These data suggest that hydrotherapy is a good potential treatment for heart failure patients. However, few studies are available, and none have compared conventional rehabilitation to hydrotherapy. This study was partially supported by “Coordenação de Aperfeiçoamento de Pessoal de Nível Superior”.

A Case Report of a Young Pregnant Woman with Preeclampsia and Acute Pulmonary Edema in Severe Transient Heart Failure

Introduction: Young pregnant woman (32 weeks) following one-month illnesses, including peripheral edema, paresthesia and shortness of breath, for which no specific diagnosis was made in the evaluations; she was hospitalized with severe respiratory distress and pulmonary edema. Re-examination revealed severe systolic heart failure (left ventricular ejection fraction: 10-15%) with preeclampsia. A...

High-Output Heart Failure Due to an Iliac Arterio-Venous Fistula in Mycotic Aneurysm

Herein, we report a patient with arterio-venous fistula secondary to mycotic aneurysm, causing high output heart failure. The patient had one-year history of refractory heart failure and recurrent pulmonary edema. This presentation is a good example of curative surgically treatable cause of high-output congestive heart failure.Iran J Med Sci 2005; 30(1): 41-44. Keywords ● Arterio-venous fistula...

Do hydrotherapy exercise programmes improve exercise tolerance and quality of life in patients with chronic heart failure? A systematic review

The purpose of this study was to evaluate whether hydrotherapy programmes improve exercise tolerance and quality of life in patients with chronic heart failure. Data sources utilised were EBSCO, Scopus, Medline, PubMed, OVID, Proquest, PEDro and Cochrane Systematic Reviews databases. A systematic review of randomised controlled trials or quasi randomised controlled trials investigated hydrother...

Is hydrotherapy an appropriate form of exercise for elderly patients with biventricular systolic heart failure?

Hydrotherapy (exercise in warm water) is considered to be a safe and beneficial method to use in the rehabilitation of stable heart failure patients, but there is little information on the effect of the increased venous return and enhanced preload in elderly patients with biventricular heart failure. We present a case of an elderly man who was recruited to participate in a hydrotherapy study. W...

Vein of Galen Aneurysmal Malformation and High-output Cardiac Failure in a Newborn

Background: Vein of Galen aneurysm (VGA) is the most common form of symptomatic cerebrovascular malformation in neonates. It develops in a fetus in the first trimester of pregnancy due to unknown reasons, but it is likely to have a genetic etiology. The prognosis of VGA is usually poor, particularly in newborns with heart failure due to high-flow intracerebral shunt. Surgery and endovascular em...
